1.      Application for Student Admission (will be provided by this office);

3.      Birth certificate of child(ren) / Family Register (translated
! p- z" y3 F! R. Hinto English);
4.      Proof of residential address / tenancy (eg lease agreement/tax
" D: o' Z# }9 w7 p- j5 H8 P5 x% Bbill/utility bill - driver’s license not acceptable);
. g1 J  v# ~# U$ F8 @$ X5.      Documentation to verify child(ren)’s first ever date of entry
into Canada (eg. passport stamp / immigration documents / airline

Study Permit,  Visitor Records and any other documents we might
require.
